PUTT is in the process of having our lawyer look over a number of “standard” contracts that the PSAOs (the companies independent pharmacies use to sign their managed care contracts) issue to their pharmacies. There has been a growing realization that the relationship between the Pharmacy>PSAO>PBM is very similar to the relationship between the Employer>Benefits Consultant>PBM. In both cases the middlemen (PSAO and Benefits Consultant) have separate and often undisclosed terms with the PBM, and in this case, what the PSAO has with the pharmacy. Further, because the documents have historically been drafted by the PSAO, it may not always be done with the PSAO acting as a fiduciary to the pharmacy.
Last year I ran across a little known company that negotiates rebates/kickbacks for the PBM’s… and of course doing so at a profit…and more overhead…

How many more middlemen are there in the mix of the PBM industry… each with their own overhead and trying to extract a profit as well…
And we wonder why Rx prices at wholesale are accelerating without any restraint !
How many entities/middlemen do we need to help us save money for the health care system ?
